For couples seeking marriage counseling or relationship counseling, each person must complete the forms.

We will go over each of these forms during our first meeting. To maximize our time together, please print, complete, and sign these forms before your first scheduled appointment. I am happy to answer questions if anything is unclear.

On the Professional Policies form, please initial next to the agreed upon fee in the "Financial Information" section on the first page.
